Every care giving opportunity is unique. That's why our professional geriatric Care Managers, credentialed experts in geriatric care, work closely with each family to understand their specific situation.

Based on a clinical assessment, the geriatric Care Manager develops a custom Plan of Care. They then work with the Caregiver and the family every step of the way to follow the plan, update it as needed and ensure compliance.

In our “Four Step Angel Plan” we follow a specific set of guidelines to develop a customized plan of care, manage that plan and bring piece of mind to the family.

Step 1: Assessment

Ability to function independently - Social isolation - Medication - Mental status - Nutrition - Home safety - Depression - Gait stability - Special needs

Step 2: Managing the Plan of Care

Communicating with family & professionals - Education oversight - Accessing best available resources - Attending to socialization needs - Ensuring home safety

Step 3: Managing the Caregivers

Interviewing Caregivers - Selecting the best Caregiver for the case - Orienting the Caregiver - Training/supervision of the Caregiver

Step 4: Serving the senior in many ways including:

Communicating with the family - Bill paying - Living arrangements - Accompanying to medical appointments - Keeping track of forms and entitlements
