Based on a clinical assessment, the geriatric Care Manager develops a custom Plan of Care. They then work with the Caregiver and the family every step of the way to follow the plan, update it as needed and ensure compliance.
In our “Four Step Angel Plan” we follow a specific set of guidelines to develop a customized plan of care, manage that plan and bring piece of mind to the family.
